To me Best Care is about improving the patient journey, consistently and in a sustainable manner.
A good example of living Best Care I have been involved with was the provision of a clear management plan for a complex patient that was agreed to by the patient, ED and ward staff. The patient had received care at the hospital about two to three times a month, and had spent over 100 days in hospital. Over the subsequent two years, prompt effective treatment remarkably reduced admissions. The patient now rarely comes to the hospital for care more than once a year and has returned to work. The patient recently commented “You have given me back my life”.
